To ensure that all of our students are eligible to play and have been cleared in all necessary areas, students are required to establish an account on https://athleticclearance.fhsaahome.org and electronically complete all required forms and requests for documentation. All forms should be uploaded onto the athlete's account.
Below are the forms required for Athletic Clearance for the 2023-2024 school year:
Traditional students, those who attend Atlantic Coast High School, must establish an account and upload the following required forms and documents:
Pre-participation Physical (EL2) – Physicals are only valid for one calendar year from the date signed by the practitioner who administered the physical. Physicals obtained during the summer will cover the student throughout the school year. **EL2 Form can be found above***
Parent Consent Form (EL3) – The Parent Consent form is only valid for one year from the date signed by the parent and student. **EL3 Form can be found above***
Sportsmanship Form – Sportsmanship forms must be completed by the student, parent and coach for each sport in which the student participates. The athletic department emphatically promotes an athletic program that practices good sportsmanship and expects good sportsmanship to be exhibited by players, coaches, parents and spectators. Birth Certificate – Each student must supply (to be photocopied) an original or certified copy of their birth certificate to the athletic department. There are no exceptions. The birth certificate need only be provided once in a student’s athletic career at Atlantic Coast.
Verification of Insurance – The FHSAA has established insurance requirements for each student and member school. Prior to participating in any team conditioning sessions, practices or games students must provide proof of insurance to the athletic department.
***Other forms may be necessary for non-traditional students who fall into the following categories: transfer students, home education, charter, private school, and international exchange students.
